### Summary
The Diabetes Program Coordinator has full responsibility for integrating evidence-based diabetes management principles into Primary Care and the Diabetes Self-Management Education (DSME) Program.
The program coordinator assures coordination of diabetes care between Primary Care, the community based outpatient clinics (CBOC's), the DSME Program, and the Endocrinology Clinic to provide optimal care and outcomes for diabetic Veterans.
The individual plans and develops policies, procedures, and goals; and implements and educates staff on applicable VHA directives, handbooks, or other policies.
The program coordinator develops, organizes, directs, manages, and controls all technical and administrative aspects of the DSME Program.
He/she is responsible for the application of the specialized principles and practices of health care management and medical nutrition therapy to assure that the program meets the national standard of accreditation for Diabetes Self Management Education of the American Diabetes Association (ADA).
The coordinator evaluates overall diabetes care through the use of metrics and reports this information to leadership.
The program coordinator serves as a primary point of contact for all diabetes education within the Medical Center and CBOC's.
Responsible for planning, implementing, directing, integrating and evaluating clinical elements and results for the Diabetes Program across the organization.
Develops and implements nutrition care protocols using principles of evidence based practice to establish and/or improve nutrition care guidelines.
Evaluates the care given to patients to assure compliance with standards.
Serves as faculty in DSME Program classes.
Practices at an advanced level of practice, and provides medical nutrition therapy.
DSME is reflected as such in their approved scope of practice for facility.
Serves as liaison or representative to National VHA committees as expert in diabetes, clinical outcomes.Determines the continuing education needs of PACT, specialty clinics and DSME Program staff and provides appropriate resources to meet those needs.
Implements and coordinates a continuing staff development program for all PACT/Specialty clinics and DSME personnel involved in caring for diabetic Veterans.
Work Schedule: 8:00 am-4:30 pm or 7:30 am-4:00 pm with rotating weekends and holidays

### Responsibilities
Ensures that all members of DSME Program meet mandatory and continuing education requirements to assure maintenance of registration status and/or the appropriate licensure.
Supports employee development by encouraging participation in promotional activities, acquiring experience in specialty areas and/or board certification.
Mentors DSME Program staff in presenting and/or publishing articles, abstracts, in professional journals or at professional meetings.
Provides program management and oversight required to develop program goals and objectives, administer and monitor the program, evaluate program outcomes/accomplishments, set and redefine priorities, and implement effective solutions in processes/systems as needed.
Monitors and evaluates Primary Care performance metrics related to diabetes care and develops and implements strategies to address those that are not meeting goal levels.
Coordinates activities of registered dietitians and other professional staff working with the DSME Program, its research, and related projects.
Provides clinical and technical advice, support and assistance to dietitians, other professionals, students, and customers in the Medical Center.
Collaborates with the assigned nursing staff to plan, develop, assess and counsel patients and with the Endocrinologist for appropriate referral and intervention of patients with severe diabetes.
Assists in the recruitment, selection, training and evaluation of clinical staff who will participate in the DSME program.
Incorporates and updates standards of care according to: VHA/DoD Diabetes Clinical Practice Guidelines, JCAHO, American Diabetes Association, Academy of Nutrition and Dietetics, EPRP, and Diabetes Education Initiatives.
Works under the administrative direction of the Assistant Chief of NFS receiving broad guidance covering the scope and mission of that division.
Functions independently in assigned patient care areas.
Supports and/or participates in clinical research studies performed in the medical center.
Develops, maintains, and supervises a system of internal reviews, direct quality control and performance improvement studies to assure compliance with various accrediting, regulatory, and agency authorities.
Performs other related duties as assigned.
Meets the needs of customers while supporting the VA mission.
Communicates and treats internal and external customers in a courteous, tactful and respectful manner.

### Qualifications
**Basic Requirements**:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* (1) The individual must have earned a bachelor's degree from a U.S. regionally accredited college or university or foreign equivalent and fulfilled each of the following criteria:
* (a) Completed a didactic program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND), formerly known as the Commission on Accreditation for Dietetic Education (CADE).
* (b) Completed an ACEND accredited or approved supervised practice program. Supervised practice programs are post-baccalaureate degree programs that provide supervised practice experiences which meet the eligibility requirements and accreditation standards of ACEND, formally known as CADE. Completion of a coordinated program in dietetics fulfills the requirements of a supervised practice program. Time spent in a dietetic internship or supervised practice program does not qualify as creditable experience.
* (2) Individuals who have earned college and/or university degrees outside the United States and its territories also must have their transcripts evaluated by a foreign transcript evaluation service to determine if their degree is equivalent to a degree granted by a regionally accredited college or university in the United States. Validation of a foreign degree does not eliminate the need for verification of completion of didactic program, dietetic internship, or coordinated program requirements. The completion of ACEND didactic program requirements and at least a bachelor's degree from a U.S. regionally accredited college/university or foreign equivalent are necessary for eligibility to enter an ACEND-accredited supervised practice program.
* 1. Ability to develop, maintain, and supervise a system of internal reviews and direct quality control and performance improvement studies to assure compliance with various accrediting, regulatory, and agency authorities.
* 2. Knowledge of the principles of program management and oversight required to develop program goals and objectives, administer and monitor program, evaluate program outcomes/accomplishments, set and redefine priorities, and implement effective solutions in processes/systems as needed.
* 3. Ability to act as a consultant for staff.
* 4. Ability to determine the continuing education needs of program staff and appropriate resources to meet those needs.
* 5. Ability to manage and establish program goals and objectives, plan and administer a program, coordinate associated activities and resources, evaluate program accomplishments, redefine priorities, problem solve, and modify objectives.
**Preferred Experience:**Applicant must have an active professional practice means paid/non-paid employment as a registered dietitian. Academic course work leading to an advanced graduate degree in nutrition or a related health care field. Clinical dietitian must have a relevant clinical nutrition healthcare experience; the food service system dietitian must have relevant food service system management experience. **Must be currently credentialed as a Certified Diabetes Educator (CDE) and maintain the CDE credentials.**
**Grade Determinations**: Completion of 1 year of experience equivalent to the next lower grade level GS-11.The full performance level of this vacancy is GS-12.
